Fix portable USB pack webroot resolution and document LAUNCH flow.

Prefer server/webroot in pack-usb (Vite outDir), cd to deck root before starting AetherForge so cwd-based webroot lookup hits usb\webroot, and note pack/LAUNCH steps in tests/README.
